Your tax forms for 2024

Tax time is here – the Internal Revenue Service (IRS) began accepting 2024 tax returns on Monday, Jan. 27, 2025. In the coming weeks, UC employees, former employees and retirees will be able to access forms verifying their 2024 income (Form W-2 and/or 1099) and health coverage (Form 1095).

Please note that you do not need to submit copies of your 1095-B and/or 1095-C form when you file your taxes; they are for your information and records only.

To access your electronic tax documents, always go to your UCPath (for employees) or UCRAYS (for retirees) account using a safe/known link. UC does not send any tax documents by email or text. If you receive an email or text with a link or an attachment for viewing your tax documents, it is a phishing scam designed to gain your private information. Do not open attachments or click email links that claim to provide access to your tax documents.

Information for current and former employees

To access your tax forms on UCPath, click on Employee Actions > Income and Taxes > View Online W-2/W-2C and/or View Online 1095. If you have questions, please visit UCPath and click on “Ask UCPath Center” to submit an inquiry or call the UCPath Center at (855) 982‐7284 from 8 a.m. to 5 p.m. (PT), Monday through Friday to speak with an associate.

W-2 forms

  • Current employees: UC employees now have access to their 2024 W-2 statements online.
    • Employees who opted for an electronic W-2 received an email notification.
    • Employees who did not opt-in for an electronic W-2 can still access their statement online via UCPath, and a printed W-2 will be mailed to the home address listed in UCPath by January 31, 2025.
    • Employees impacted by UC Health acquisitions will receive two W-2s: One from your previous employer and one from UC.
  • Former employees: You may view or download your W-2 statements on UCPath for up to three years after your separation date.

1095 forms

  • Current employees: If you signed up before Jan. 1, 2025, UCPath will email you when your 1095 form(s) are available for download. If you didn’t request electronic delivery, you should expect to receive your 1095 forms over the next few weeks, depending on the timing of US Postal Service (USPS) delivery. Except for Residents & Fellows Benefits members, all employees can now access a duplicate copy of their 1095 forms through UCPath, along with 1095 forms from 2024 and 2023. All other previous years are not available electronically but can be requested for printing and mailing.
  • Former employees: Former employees will have access to an electronic duplicate copy of their Form 1095, reflecting your eligibility and/or enrollment in UC-sponsored medical plans.
  • Former employees continuing coverage by COBRA: Former employees who continued coverage through COBRA will receive a separate Form 1095 reflecting your COBRA coverage. UC Care, CORE and UC Health Savings Plan members will be sent a paper copy of your 1095 form. You should expect to receive your 1095 forms over the next few weeks, depending on the timing of USPS delivery. Members of Kaiser Permanente and UC Blue & Gold HMO will receive a paper form directly from your health insurance carrier (Kaiser or Health Net). No electronic copies will be available on UCPath.

Information for retirees

It’s easier than ever to access your tax documents from your UC Retirement At Your Service (UCRAYS) account. When you view your tax statement(s), you will be sent an email to confirm it was a valid request and you will be required to verify your identity by entering a one-time code sent to you via text or email.

If you need help logging in to your UCRAYS account, just call the UC Retirement Administration Service Center at 800-888-8267, Monday–Friday, 8:30 a.m. to 4:30 p.m. (PT), and select option 1. To minimize wait times, UnifyHR (a trusted UC partner) is providing additional support for UCRAYS issues.

1099-R statements

Select “Tax Statements” on your UCRAYS dashboard to view or download your 2024 tax statements. If you requested US Mail delivery, your printed form was mailed to the address you have on file as of January 2025; you also have the option to view a duplicate copy on UCRAYS.

1095-B forms

  • UC Care, CORE and UC Health Savings Plan: The 1095-B form will be available in UC Retirement At Your Service (UCRAYS) and will be available for download. If you opted-in for electronic distribution of all tax documents, you will not receive a paper copy of your 1095-B form. If you opted out of electronic distribution of your tax documents, your 1095-B form will be mailed to your home address. All tax documents, including the 1095-B form, will be available in UCRAYS and will be available for download beginning the week of January 27, 2025.
  • Kaiser Permanente and UC Blue & Gold HMO: You will receive a paper form directly from your health insurance carrier (Kaiser or Health Net).
  • UC Medicare plans: The Centers for Medicare and Medicaid Services (CMS) is the responsible entity for issuing 1095 forms for all individuals on Medicare, including UC retirees enrolled in UC-sponsored Medicare plans or in Medicare plans through Via Benefits. If you had Medicare Part A coverage for part of 2024, you will have a 1095-B form mailed directly to you from CMS. At this time, it is unknown when the mailings are expected to be released. If you had Medicare Part A coverage for all of 2024, you will not receive 1095-B forms from CMS.

Reprints and corrections

For employees and former employees, duplicate copies of Form 1095 are available in UCPath. For retirees, duplicate copies of 1095 and 1099-HC forms are available on UCRAYS.

To request a correction to your 1095 based on your UC employment, submit that request to UCPath by scrolling down to the Forms Library and clicking on “Records” to complete a Benefits record request.

To request a correction to your 1095 generated by UC as a retiree, submit that request to RASC.

If you were provided a 1095 that was provided to you by Health Net or Kaiser in 2024, reach out to your health plan for reprints or corrections to Form 1095-B.
