Annual Health Net satisfaction survey

If you are a member of UC Blue & Gold, Health Net will be reaching out soon by email (see screenshot below for reference) with a three-question survey to learn more about your experiences.
These three questions are:
- How likely is it that you would recommend Health Net to a friend, family member or colleague? This score ranges from 0 (not at all likely) to 10 (extremely likely)
- What is the most important reason for your score?
- If you left a comment above, please tell us what it was mostly about.
This survey will not ask for your name, username/password, phone number or other personal information.
