Your tax forms for 2025

Tax time is here – the Internal Revenue Service (IRS) began accepting 2025 tax returns on Monday, Jan. 26, 2026. UC employees and former employees can now access forms verifying their 2025 income (W-2 Statement) and health coverage (Form 1095-B/1095-C) on UCPath.
Please note that you do not need to submit copies of your Form 1095-B/1095-C when you file your taxes; they are for your information and records only.
To access your electronic tax documents, always use a safe/known link to your UCPath account. UC does not send any tax documents by email or text. If you receive an email or text with a link or an attachment for viewing your tax documents, it is a phishing scam designed to gain your private information. Do not open attachments or click email links that claim to provide access to your tax documents.

To access your tax forms on UCPath, go to Income & Taxes > View Online W-2/W-2c and/or View Online 1095-C. If you have questions, please visit UCPath and select “Ask UCPath” to submit an inquiry or call UCPath at (855) 982‐7284 from 8 a.m. to 5 p.m. (PT), Monday through Friday, to speak with an associate.
W-2 Statements
- Current employees: UC employees now have access to their 2025 W-2 statements online.
- Employees who opted for an electronic W-2 statement received an email notification. Employees who did not opt-in for an electronic W-2 statement will receive a printed W-2 statement at the mailing address listed in UCPath. W-2 statements were mailed on January 30, 2026. You can still access your W-2 statement(s) online via UCPath.
- Employees who transitioned to UC in 2025 will receive two W-2 statements: One from your previous employer and one from UC.
- Former employees: You may view or download your W-2 statements on UCPath for up to three years after your separation date.
Form 1095-C
- Current employees: If you signed up before January 2, 2026, UCPath will email you when your Form 1095-C is available for download. If you didn’t request electronic delivery, you should expect to receive your Form 1095-C over the next few weeks, depending on the timing of USPS delivery. Except for Residents & Fellows Benefits members, all employees can now access a duplicate copy of their Form 1095-C through UCPath, along with Forms 1095-C from 2024 and 2023. All other previous years are not available electronically but can be requested for printing and mailing.
- Former employees: Former employees will have access to an electronic duplicate copy of their Form 1095-C, reflecting your eligibility and/or enrollment in UC-sponsored medical plans.
- Former employees continuing coverage by COBRA: Former employees who continued coverage through COBRA will receive a separate Form 1095-C reflecting your COBRA coverage. UC Care, CORE and UC Health Savings Plan members will receive a paper copy of your Form 1095-C. You should expect to receive your Form 1095-C over the next few weeks, depending on the timing of USPS delivery. Members of Kaiser Permanente and UC Blue & Gold HMO will receive a paper form directly from your health insurance carrier (Kaiser or Health Net). No electronic copies will be available on UCPath.
Form 1095-B
UC Care, CORE and UC Health Savings Plan: The Form 1095-B will be available in UCRAYS for download. If you opted-in for electronic distribution of all tax documents, you will not receive a paper copy of your Form 1095-B. If you opted out of electronic distribution of your tax documents, your Form 1095-B will be mailed to your home address. All tax documents, including the Form 1095-B, will be available in UCRAYS and will be available for download beginning the week of January 5, 2026.
Kaiser Permanente and UC Blue & Gold HMO: You will receive a paper form directly from your health insurance carrier (Kaiser or Health Net).
Reprints and corrections
For employees and former employees, duplicate copies of Form 1095-C are available in UCPath.
To request a correction to your Form 1095-C based on your UC employment, go to UCPath, click on Personal Information > Records Request eForm and complete a benefits records request.
If you were provided a Form 1095-B by Health Net or Kaiser in 2025, reach out to your health plan for reprints or corrections.
