UC Health shares progress-filled population health report

University of California Health has released its FY 2024–2025 UC Population Health Annual Report. This overview highlights the significant progress across UC Health’s population health initiatives — including key initiatives and collective accomplishments — as the UC academic health centers continue to advance value-based care in California with the support of the UC Population Health team.

The initiatives outlined in the report address many important aspects of California communities’ health care needs and overall well-being. The new value-based Care Administrators initiative, launched in 2024, is helping to advance UC’s health system in providing value-based care by understanding and sharing the elements of successful participation in governmental and commercial alternative payment models. The report highlights this work and other major efforts in more detail.
