The University of California embraces diversity in the workplace and is committed to providing a supportive environment for all employees. This October, as we celebrate National Disability Employment Awareness Month, we invite UC staff to update the Voluntary Self-Identification of Disability Form.
Self-identification is voluntary and confidential. It is not used as the basis for any employment decisions and every employee can select the “I DO NOT WISH TO ANSWER” option. The information we collect helps us ensure that UC locations are accessible and welcoming to persons with disabilities. It also allows us to have the most accurate data about our employment practices and recruitment efforts.
For more information about self-identifying a disability and other demographic information please see Frequently Asked Questions for Academic & Staff Employees about Self-Identification Surveys (PDF) on the UC Office of the President website.
