Family member eligibility verification

To ensure UC’s benefits resources are used wisely, anyone who enrolls new family members in their health and welfare benefit plans must provide documents to verify their family members’ eligibility for coverage. In 2025, UC will also reach out to confirm the continuing eligibility of some previously verified family members. 

UnifyHR administers the verification and reverification program for UC.

Understanding the process

Employees and retirees who enroll new family members during the year or during Open Enrollment will receive a packet of materials from UnifyHR to help them complete the verification process. In 2025, UC will also reach out to confirm the continuing eligibility of some previously verified family members. 

You will receive a packet of verification materials from UnifyHR:

  • In early 2025 if you enrolled a new family member or members in your UC coverage during Open Enrollment
  • At any time throughout the 2025 plan year if you completed verification more than 3 years ago for the following dependents: spouse, domestic partner, stepchild, grandchild/step-grandchild, legal ward or overaged disabled child

You must respond by the deadline shown on the letter to protect your family members’ coverage. Help will be available if you have any questions or concerns about the process.

If UnifyHR contacts you

UnifyHR will reach out by mail if they require additional documentation to establish your dependents’ eligibility for coverage. They will send an email and make one phone call if they have not received any documents by a certain date. You will receive a letter of cancellation if your dependents are being disenrolled from UC benefits.

UnifyHR representatives can answer your questions and let you know what documentation they have on file for you and what documentation they need. When you call, be sure to have any letters you’ve received from UnifyHR handy, so you can refer to them during your call.

Multi-factor authentication (MFA) on UnifyHR portal

To increase the security of employee and retiree data, the UnifyHR portal implemented Multi-Factor Authentication (MFA) on October 1, 2024. MFA adds an extra layer of security by verifying a user’s identity before access is granted. When logging into the UnifyHR portal, you will be prompted to enter an 8-digit code sent to your email address.
