UC requires all faculty, staff and retirees who enroll new family members in their medical, dental and/or vision insurance plans to provide documents to verify their family members’ eligibility for coverage.

If you add a family member to your coverage during Open Enrollment, in early 2018 you will receive a packet of materials to complete the verification process. You must respond by the deadline shown on the letter or risk de-enrollment from benefits for you and your newly enrolled family members. No documentation is required for family members whose eligibility was verified previously.
